RESUMO

Halogenation of pyrrole requires strong electrophilic reagents and often leads to undesired polyhalogenated products. Biocatalytic halogenation is a highly attractive approach given its chemoselectivity and benign reaction conditions. While there are several reports of enzymatic phenol and indole halogenation in organic synthesis, corresponding reports on enzymatic pyrrole halogenation have been lacking. Here we describe the in vitro functional and structural characterization of PrnC, a flavin-dependent halogenase that can act on free-standing pyrroles. Computational modeling and site mutagenesis studies identified three key residues in the catalytic pocket. A moderate resolution map using single-particle cryogenic electron microscopy reveals PrnC to be a dimer. This native PrnC can halogenate a library of structurally diverse pyrrolic heterocycles in a site-selective manner and be applied in the chemoenzymatic synthesis of a chlorinated analog of the agrochemical fungicide Fludioxonil.

RadH is one of the flavin-dependent halogenases that has previously exhibited promising catalytic activity towards hydroxycoumarin, hydroxyisoquinoline, and phenolic derivatives. Here, we evaluated new functional homologs of RadH and expanded its specificities for the halogenation of non-tryptophan-derived, heterocyclic scaffolds. Our investigation revealed that RadH could effectively halogenate hydroxyquinoline and hydroxybenzothiophene. Assay optimization studies revealed the need to balance the various co-factor concentrations and where a GDHi co-factor recycling system most significantly improves the conversion and efficiency of the reaction. A crystal structure of RadH was also obtained with a resolution of 2.4 Å, and docking studies were conducted to pinpoint the binding and catalytic sites for substrates.

Chronic postsurgical pain (CPSP) occurs in up to 50% of individuals after surgeries and 32% after hysterectomy, leading to major adverse effects on quality of life and socioeconomic burden. Little is known about whether and how large-scale neural networks being affected in CPSP, particularly with regard to the functional connectivity (FC) of insula which is known to be the hub of the intrinsic neural network playing a critical role in pain processing. Here, we sought to examine the dynamics of insular FC in the context of noxious stimuli in CPSP patients. To this aim, resting state fMRI data were acquired, before and after acute heat pain stimulation, from 11 individuals with chronic post-hysterectomy pain (CPHP) and 22 age-matched healthy controls (HCs) who had a hysterectomy but without chronic post-surgical pain. We examined whole-brain FC were mapped by seeding at the sensorimotor and chemosensory subfields of the insula and found significant group × stimulation interaction effects. Specifically, the HC group had increased FC between the left sensorimotor insula and right angular and middle occipital gyrus (MOG) and increased FC between the left chemosensory insula and bilateral angular and MOG following pain stimulation. In contrast, such pain stimulation related FC changes were absent in the CPHP group. Furthermore, higher insular FC at baseline and smaller increased insular FC after pain stimulation correlated with clinical pain scores in CPHP patients. Our findings suggest that CPSP is associated with altered dynamics of large-scale functional networks anchored in the insula.

BACKGROUND: Marine stings and envenomation are fairly common in Malaysia. Possible contact to various marine life occurs during diving, fishing and food handling. Even though majority of fish stings are benign, there are several venomous species such as puffer fish, scorpion fish, lionfish, stingray and stonefish that require urgent medical treatment. Stonefish is one of the most venomous fish in the world with potential fatal local and systemic toxicity effects to human. CASE PRESENTATION: We reported a case of stonefish sting complicated with impending compartment syndrome. CONCLUSIONS: Medical staff should be alert about the possibility of this potential emergency in standard management of stonefish stings.

BACKGROUND: A comparative study between two treatment methods (collagenase injection and open partial fasciectomy) for Dupuytren's contracture. This study will determine differences in clinical outcome, complication rate and patient satisfaction. METHODS: 37 patients with 62 metacarpophalangeal joints (MCP) and 44 proximal interphalangeal joints (PIP) treated. There were 21 MCP joints (34%) and 8 PIP joints (18%) treated with injection. The remaining 66% of MCP joints and 82% of PIP joints were treated by open partial fasciectomy. RESULTS: Overall, both treatment methods were successful in correcting the passive extension deficit in the MCP and PIP joints. Minor complications were reported in 45% of patients in the injection group versus 42% in the surgery group. Patient satisfaction was nearly equal for both groups. CONCLUSIONS: Both treatment options have proven their effectiveness in treating Dupuytren's contracture. Open surgery is able to address additional joint contracture problems commonly associated with Dupuytren's disease. Collagenase injection has the advantage of early return of hand function and avoidance of surgical complications.

Chondrogenic differentiated mesenchymal stem cells (CMSCs) have been shown to produce superior chondrogenic expression markers in vitro. However, the use of these cells in vivo has not been fully explored. In this study, in vivo assessment of cartilage repair potential between allogenic-derived chondrogenic pre-differentiated mesenchymal stem cells and undifferentiated MSCs (MSCs) were compared. Bilateral full thickness cartilage defects were created on the medial femoral condyles of 12 rabbits (n = 12). Rabbits were divided into two groups. In one group, the defects in the right knees were repaired using alginate encapsulated MSCs while in the second group, CMSCs were used. The animals were sacrificed and the repaired and control knees were assessed at 3 and 6 months after implantation. Quantitative analysis was performed by measuring the Glycosaminoglycans (GAGs)/total protein content. The mean Brittberg score was higher in the transplanted knees as compared to the untreated knee at 6 months (p < 0.05). Quantitative analysis of GAGs was consistent with these results. Histological and immunohistochemical analysis demonstrated hyaline-like cartilage regeneration in the transplanted sites. Significant differences between the histological scores based on O'Driscoll histological grading were observed between contralateral knees at both 3 and 6 months (p < 0.05). No significant differences were observed between the Britberg, O'Driscoll scores, and GAGs/total protein content when comparing defect sites treated with MSC and CMSC (p > 0.05). This study demonstrates that the use of either MSC or CMSC produced superior healing when compared to cartilage defects that were untreated. However, both cells produced comparable treatment outcomes.
